Be You Support Coordination is a registered provider that started in the Central Coast NSW in 2021, built with the focus on delivering the service we are most passionate about. Our tailored supports aim to provide the highest level of Support Coordination, while supporting other engaged providers to ensure best possible supports and focus on goal achievement. Since the beginning of BYSC we have continued to expand our team and focus on ensuring we provide the highest level of support to meet the needs of all.

Our team has extensive experience in Mutli Disciplinary Supports and Complex Coordination. The BYSC team have extensive experience with the following teams or departments (however they are not limited to the following) Department of Housing, Department of Health and Aged care, Mental health supports, Youth supports, local and state initiatives, Specialized medical teams throughout NSW and VIC, Domestic Violence supports.

Our passion and dedication to Support Coordination and the industry comes from decades of experience within the industry both locally and nationally. As a team this provides us with extensive experience, knowledge, and extensive skills in the following:

• Multidisciplinary Case management

• Effective implementation of complex supports

• Reviewing quality of support plans and evidence

• Mitigating Crisis and changing needs

• Working collaboratively with open communication and transparency

• Connection and information to assist in informed decision making to create a support team to meet the needs and work to achieve goals

• Accessing NDIS funded accommodation

• Experience in supporting individuals within the NDIA funded accommodation setting

• Mental Health comorbidities

• Community and mainstream services and supports

• Complex health needs and treatment plans 

• Complex treatment plans within disability, behaviors, mental health, health and other areas for management
